The Ohio Department of Health (ODH) mandates the immunizations necessary for students attending school in Ohio. Written proof of the dates the vaccinations were administered is required by your child’s health care provider at the start of the 2021-2022 school year. Students without adequate proof of these vaccinations will be subject to exclusion. If you choose not to immunize your child, then an objection form must be completed. Required immunizations for all incoming seventh grade students:

• One dose of the tetanus, diphtheria, and pertussis (Tdap) booster.

• First dose of the meningococcal vaccine (MCV4).

Required immunizations for all incoming twelfth grade students:

• Two doses of the meningococcal vaccine (MCV4). If the first dose of MCV4 was administered on or after the 16th birthday, a second dose is not required.
