The Ohio Department of Health (ODH) mandates the immunizations necessary for students attending school in Ohio. The Meningococcal vaccine (MCV4) is a NEW required vaccine for all Ohio students entering the seventh and twelfth grade for the 2016-2017 school year. Written proof of the dates the vaccinations were administered is required by your child’s health care provider at the start of the 2016-2017 school year. Students without adequate proof of these vaccinations will be subject to exclusion. If you choose not to immunize your child, then an objection form must be completed. If you have any questions, please call the nurse supervisor at 576-4731 ext: 1351.

Required immunizations for all incoming seventh grade students:

  • One dose of the tetanus, diphtheria, and pertussis (Tdap) booster.
  • First dose of the meningococcal vaccine (MCV4). This is a NEW required vaccine for the 2016-2017 school year.

Required immunizations for all incoming twelfth grade students:

  • Two doses of the meningococcal vaccine (MCV4). If the first dose of MCV4 was administered on or after the 16th birthday, a second dose is not required.
